Nina the New Nurse finds herself in a predicament not covered in nursing school. A coworker, another RN, was late for work. When the nurse arrived to the unit, she appeared disheveled and unsteady on her feet. As Nina the New Nurse gives the nurse a short report of her clients, she smells a strong scent of breath mints. Nina the New Nurse is certain the nurse is intoxicated; however, she is not certain what she should do. Nina the New Nurse feels concerned about the safety of the nurse’s clients. However, she feels a bit intimidated; what if her suspicions are wrong? Which one or more of the following actions should Nina the New Nurse take?
  1. Nina the New Nurse should keep quiet; she does not know for sure the nurse is intoxicated and really should not get involved.
  2. Nina the New Nurse should notify the supervisor of her suspicions.
  3. Nina the New Nurse should talk to the other nurses about the nurse in question. A more experienced nurse will know what to do.
  4. Nina the New Nurse should not allow the nurse to perform her nursing duties.
Explanation
Answer: B and D - Client safety is always the priority. Notifying the supervisor of the situation is necessary; however, immediately protecting the client from possible harm is priority. Risking client safety to protect an impaired nurse is a violation of nursing ethics. While discussing the situation with other nurses, the impaired nurse could cause harm to a client. A substance-impaired nurse is not competent to perform client care.
