Answer - C - The patient care continuum begins when treatment is initiated in the field and extends into the emergency department. This continuum is made possible by effective communication between pre-hospital personnel and the emergency department staff. Failure to communicate breaks this continuum and ultimately compromises patient care. The omission in relaying current medical care or medication can affect the way a patient is treated or evaluated in the emergency department; therefore, there must be responsibility for the patient via the patient care continuum.
