While filling out a report you make an error; how do you correct the error properly?
  1. Use white out to cover the error and write the correct information on top of the white out.
  2. Draw a single line through the error, initial & date it, then write in the correct information beside it.
  3. Scribble out the error so it is illegible, and then write in the correct information next to it.
  4. Throw the report away and start over with a clean report sheet.
Explanation
Answer: B - If you make an error while filling out a report sheet, simply draw a single line through the error, initial and date it and write the correct information beside it. By trying to cover an error you can make it look like you are trying to hide something even though you are not. In computerized charting, your report is finalized when uploaded to the system and further changes require that you create an additional document to be attached to the original containing the amended information and reason for changes.
