The NP is evaluating the 12 lead electrocardiograph of a patient who had a myocardial infarction approximately 2 hours ago. The doctor stated the MI was still “evolving.” What abnormalities would the NP expect to see to reflect the ischemic event?
  1. ST segment elevation
  2. A widening of the QRS interval
  3. ST segment depression
  4. The emergence of Q waves
Explanation
Answer: A - The NP would expect to see the abnormal elevation of the ST segment as a reflection of the ischemic event. The emergence of Q waves would indicate worsening ischemia and necrosis.

Julie Darcy says:

It was my understanding that new ischemia was indicated by ST depression. When an infarct is ongoing, you get ST-elevation, and once you get necrosis, you get Q-waves.
