Nina the New Nurse is at the nursing station. A G.I doctor has a signed informed consent for an EGD and asks Nina the New Nurse to witness the client’s signature. She wants to be helpful, so she witnesses the client’s signature. What (if anything) did Nina the New Nurse do wrong?
  1. Nina the New Nurse did not do anything wrong.
  2. Nina the New Nurse should have refused.
  3. Nina the New Nurse should have confirmed with the client that the signature on the informed consent form was indeed his and confirmed that the client understood what he was agreeing to.
Explanation
Answer: C - A nurse may witness the client signature of an informed consent. By signing as a witness, the nurse is verifying s/he witnessed the actual client is giving consent for the procedure, has not been pressured into giving consent, has the ability to understand the risks/benefits of the procedure. Facilities may want the physician to obtain consent, while others may allow the nurse to get the consent signed. It is always the nurse’s role as client advocate to assess the clients understanding of what he has agreed to.
